The unsustainable cost of healthcare in the United States is due significantly to very expensive hospital care and challenges brought on by longer life expectancy and the prevalence of chronic illnesses. Innovative strategies are required to increase individual and community competencies for self management and social action is necessary to ensure healthcare for all including vulnerable and minority populations.
